This doesn't only happen with HNMs/Aspid/Behe/etc, but regular NMs too. (Where they flash or appear/disappear very quick and pop next window) Xol does this alot and even NMs like LL.

Makes me nervous about killing NMs sometimes. This has been happening a lot lately but is inconsistent. Like you said with HiB, I think a lot of that is lag-related. Just my two cents.

I have seen some mins go white in the past when dealing with ramps/stairs. Most notably stairs in Temple Ugg. However in these instances the mobs were also "out of range" and disappeared from mob radar leading me to believe that they were somewhere well off the normal Z Axis.

I've never had it happen while I was able to actively swing on a mob while solo.

I'm not saying I don't believe the report but I'd love to see a video for science

That's a heavy possibility, the z-axis is buggy as shit, paired with mob movement being buggy as shit, definitely could be the mob twitches into the right spot, just enough to shoot it's z axis out of aggro range for a sec....

I'm pretty sure that when I saw the issue in Saruta with regular ol' Mandragora that I was fighting them on the small hills and junk near the roads, so there's a definite possibility of z-axis-related shenanigans.
