I don't fully know. I was on windows 7 and I tried 2 weeks of "fixes" and nothing stuck.
When I started monitoring it, the amount of GPU usage dropped a LOT when the spikes started to happen and the CPU usage raised significantly. It felt as though the usage was almost just transferred from the GPU to the CPU. This is what was causing the game to start to have violent stutters once this "kicked in".
I honestly don't even know how that is possible.
I tried something extremely out there since nothing else worked.
I upgraded my windows 7 to windows 10 and the problem went away. I still had the basic stutter from controller being enabled, but I disabled that right away on Win 10 and it was good to go. The problem hasn't happen since.
I don't understand why this issue happened on Win 7 and not Win 10.
As a final note: No hardware changed, no settings changed, absolutely everything was the same and yet no problems after upgrading. So strange...
