I just did mine and posted it on another thread but I'll bring it onto this one as well lol.

I just made an alt finally and I have never dualboxed in my life. So this is completely new to me and I have not got the alt*tab thing down yet. Anyway, in the dunes in a party with my 16 subjobless blm and my 64 Blu helping PL. There is that gob that ones all over the lizzie camp and I would go kill it with my BLU so we didn't have to worry about. Well, I go pull my weapon out and attack it only to realize I was on the wrong character and attacked with my 16 BLM instead. Ended up wiping out the entire party. I felt so bad but they all were good sports and took it in stride. We ended up having a great party with no more dumb mistakes lol.

I was to open a mission fight with ES Sleepga, but hit Sleep by mistake. Everything instantly went to shit and we wiped.

I struggle going into action cold, like in an instanced fight where you go straight into the battle with no warm up. I need to get my mind into the rhythm of buffing, healing, debuffing and judging what is currently the top priority.

not even kidding, i turn off the music when i go into a BCNM type battle. It super intense! i get way too anxious and i feel like i make mistakes.

- Sometimes I sleep too early in lowman dyna and wipe the entire group (but hey, doesn't everyone every once in a while?).

- I've killed the wrong mob in Limbus a couple times (after very carefully checking to make sure I was fighting the right mob, then entirely ignoring the carefully-compiled output of my brain), causing us to have to start the entire floor over again. Of course, this is usually when there are like 3 other groups trying to take turns clearing floors, cuz that's just the way it goes.

- On the same token, I've traded an Omega pop while there was already fight going on without checking myself that the zone was clear (we'd had Apollyon to ourselves for hours, didn't even think to double-check before we all said 'go'.. but meh, if you're holding the chips, you should be the one to double-check).

- The number of times I've died because I wasn't paying close-enough attention to my alt is kinda silly. I feel I've gotten better at it over time, but I definitely don't consider myself the server's greatest dualboxer.

- I shared unholy knowledge of forbidden fruit with the wrong crowd (rather than just taking it straight to you Nas, sorry man) and enabled a group of people to royally fuck up the server's economy for months (again, in at least one case) and got all of them (at least one for like the 3rd time) and myself jailed in the process, woo!

I think I've shared this before, but it's one of the things that stick out in my head. Back on retail when I was a lower level, I was getting help on missions or something from high level people. I brought sneak and invis pots and thought they'd be impressed at how prepared I was. When it came time to use sneak, I popped the potion. Turns out I bought silencing potions, not silent oils.

We're wondering what's going on. I quickly realize I entered the wrong one, and did not wait for the guy to trade the Ark.
My group, who's in the wrong BCNM is watching the health of the split group drop quick.
I kept the fuck up to myself and shifted blame to #nasomithings.

As a NIN main, I've 2hr'd an unhealthy amount of times on accident in EXP parties.

Frequently I forget to get a warp scroll and can't figure out the issue for sometimes hours. The issue ends up being either A) I put it in my MH, or B) My inventory is full.

I accidentally open the Sales tab on AH a lot. Can be a real pain in the ass to re-zone when you're in a rush.

I've lost probably 100-200k+ in lures for fishing. I forget which character I'm on a lot and will press a button to pull early; sometimes it's just a delay with SwitchMon that screws me.

I saw the staff market crashing before it happened. Made a fairly large investment in "cheap" Iridals. I ended up making -120k on every staff. Would rather not disclose how many staves I bought.

Just recently I thought it'd be okay to go into RDM Maat fight with 150 Elemental skill. False.

I'm drawing a blank for anything major; usually if I mess up in a group setting I end up being lucky with the outcome, but I have hard standards for trying to fuck up as little as possible.

Some* of my mistakes can be blamed on Mary Jane.
